Taking Clomid can lead to an increase in testosterone and sperm count in some men. Dr. Tieche uses this in some cases for men with Low T when Testosterone is not desired.

Clomid is the brand name for clomiphene citrate, an oral prescription medication that may be used in certain men with low testosterone or fertility concerns.

Unlike testosterone replacement therapy, which provides testosterone from outside the body, Clomid may help stimulate the body’s own testosterone production. For this reason, it may be considered for men who want to improve testosterone levels while preserving natural testicular function and sperm production.

Clomid is FDA-approved for ovulatory dysfunction in women. Its use in men is considered off-label, meaning a qualified provider may prescribe it when medically appropriate even though male treatment is not included in its FDA-approved labeling.

How Does Clomid Work?

The brain and testicles work together to regulate testosterone production. Two important signaling hormones are:

  • Luteinizing hormone, or LH, which signals the testicles to produce testosterone
  • Follicle-stimulating hormone, or FSH, which helps support sperm production

Clomid blocks some of estrogen’s feedback signals to the brain. In certain men, this may encourage the pituitary gland to release more LH and FSH, which can help the testicles produce more testosterone and may support sperm production.

Clomid does not work for every type of low testosterone. Its effectiveness depends on why testosterone levels are low and whether the testicles are able to respond appropriately.

Who May Be a Candidate?

A Recharge Clinic provider may consider Clomid for men who:

  • Have symptoms of low testosterone with supporting laboratory results
  • Want to preserve fertility or sperm production
  • Are actively trying to conceive
  • Prefer to stimulate natural testosterone production rather than begin traditional testosterone therapy
  • Have certain forms of secondary hypogonadism, in which the brain is not adequately signaling the testicles
  • Need an alternative based on their medical history or treatment goals

Clomid is not technically testosterone replacement therapy, and it should not be described as a guaranteed substitute for TRT. Some men respond well, while others may need a different treatment approach.

Clomid, Testosterone Therapy and Fertility

Traditional testosterone therapy can reduce the hormonal signals responsible for sperm production. Clomid may be considered when fertility preservation is important because it works by stimulating the body’s internal signaling system rather than supplying external testosterone.

Evaluation and Monitoring at Recharge Clinic

  • Your symptoms and health history
  • Current medications and supplements
  • Total and free testosterone
  • LH and FSH
  • Estradiol
  • Prolactin and thyroid testing when appropriate
  • Blood counts and other health markers
  • Semen analysis when fertility is a concern

Follow-up laboratory testing is important to determine whether treatment is working and whether adjustments are needed.

Possible side effects may include headaches, mood changes, changes in vision, breast tenderness, increased estrogen levels or other hormonal symptoms. Contact your provider promptly if you develop visual changes or another concerning reaction.
