What NAD Therapy Actually Feels Like: The Truth About NAD, Burnout, Recovery, and Why Some Patients Swear By It

May 29, 2026

By Steve Tieche

By now you’ve probably heard NAD described online as:

  • the fountain of youth
  • a miracle molecule
  • biohacker fuel
  • celebrity anti-aging therapy
  • “liquid energy”

Which is unfortunate.

Because whenever something in medicine gets hyped hard enough online, people start expecting fireworks, laser beams, spiritual enlightenment, and immediate abs at the end of an IV infusion.

That’s not real life.

NAD is not magic.

But I’ll also tell you this honestly:

Some patients absolutely love it.

And when used appropriately, in the right patient, as part of a bigger plan… it can be a very powerful tool.

The Patient I Still Think About

A while back, I had a patient come in who looked completely drained.

Not dramatic. Not lazy. Not unhealthy on paper. Just… depleted.

Successful professional. Busy parent. High stress. Sleeping poorly. Running on caffeine and adrenaline for years.

The kind of person who keeps functioning because stopping isn’t an option.

When I asked how they felt, they said:

“I don’t even know how to explain it. I feel like my brain and body just stopped keeping up with me.”

That sentence stuck with me. Because honestly, I hear versions of that almost every day now.

They had already tried:

  • supplements
  • hormones
  • better diet
  • exercise
  • sleep apps
  • enough caffeine to tranquilize a horse

Some things helped, but they still felt flat.

Foggy. Exhausted in a way that sleep didn’t seem to fix.

We ended up building a much bigger plan:

And no, the patient didn’t leap off the IV chair screaming:

“I HAVE ASCENDED TO A HIGHER DIMENSION.”

That’s social media nonsense. What actually happened was more interesting.

A few weeks later they came back and said:

“I feel sharper.”
“I’m not crashing at 2 PM anymore.”
“I’m handling stress better.”
“I actually want to work out again.”
“I feel more like myself.”

That’s the phrase I hear the most with successful NAD patients:

“I feel like myself again.”

And honestly? That’s a pretty big deal.

NAD Is Not Caffeine

This is one of the biggest misconceptions people have.

NAD is not supposed to feel like:

  • pre-workout
  • an energy drink
  • an amphetamine
  • ten espresso shots and a motivational speech

If that’s what someone expects, they’re going to misunderstand the entire point of it.

NAD is about supporting cellular energy production.

That’s a completely different concept than artificial stimulation.

Caffeine borrows energy against tomorrow.

Cellular support is about helping the system function more efficiently long-term.

Very different conversation.

What Does NAD Therapy Actually Feel Like?

This varies wildly from person to person.

Some people notice:

  • better mental clarity
  • smoother energy
  • less burnout feeling
  • improved recovery
  • better focus
  • more resilience under stress
  • improved motivation

Others notice subtler shifts:

  • waking up easier
  • less afternoon crashing
  • improved workouts
  • better consistency

And some people notice almost nothing initially… until other major issues are addressed too.

Because here’s the truth nobody on Instagram wants to tell you:

You cannot out-IV a broken lifestyle.

If someone:

  • sleeps 4 hours
  • drinks excessively
  • lives on ultra-processed food
  • has untreated sleep apnea
  • ignores their hormones
  • never recovers
  • runs on stress constantly

…then no IV on earth is going to transform them into a superhero.

That’s not medicine. That’s fantasy.

The Real Conversation Is Burnout

I think one reason NAD resonates with people right now is because modern society is absolutely cooking human nervous systems.

People are overwhelmed.

Mentally overloaded.
Physically depleted.
Constantly stimulated.
Never recovering.

And many people don’t even realize how bad they feel until they finally start feeling better.

That’s the scary part.

They normalize exhaustion.

Not Everyone Needs NAD

I think ethical medicine means saying this clearly.

Not every person needs NAD therapy.

Some people actually need:

Sometimes NAD is part of the plan. Sometimes it’s not. And that’s okay.

Good medicine should never become:

“Everybody gets the same trendy protocol.”

Why Medical Oversight Matters

NAD therapy is not something I recommend people blindly experiment with because they watched a podcast clip at 1:30 in the morning.

There are real considerations:

  • dosing
  • infusion speed
  • tolerance
  • nausea
  • headaches
  • patient selection
  • underlying medical conditions

Some people tolerate aggressive protocols well. Others need slower, more gradual approaches. That’s why individualized medicine matters.

At Recharge Clinic, we try to evaluate the whole patient:

  • labs
  • symptoms
  • hormones
  • sleep
  • metabolic health
  • stress
  • recovery
  • lifestyle

Because health is rarely one-dimensional.

My Honest Perspective on NAD

I don’t think NAD is fake hype.

I also don’t think it’s magic.

I think it’s a fascinating and potentially powerful tool in the right context. And I personally use NAD injections weekly along with many other treatments to keep myself optimized and feeling youthful.

And I think one of the most important shifts happening in medicine right now is that we’re finally talking more about:

  • cellular health
  • mitochondrial function
  • recovery
  • resilience
  • prevention
  • optimization

Instead of waiting until people completely crash before paying attention.

Final Thought

A lot of people aren’t lazy. They aren’t weak. They aren’t “just getting older.” Sometimes their system has simply been running depleted for far too long. And sometimes helping the body recover at a deeper level can change far more than people expect.
